We are based at Milborne Port Surgery and work under the guidance of your named GP.

The aim of our role is for us to work together to help you identify what choices are available to you, regarding your health and life.

We work with patients – enabling and supporting them to make the changes needed to benefit their health. We will work together to set goals which will help you manage your health and help you identify the support network that is required for you. We support selected patients referred to us, this could be help for the patient or family members coping with a situation where they may need some support or signposting to specialist services for support.

We are non-clinical.

We help and encourage people to gain knowledge, skills and confidence through positive changes.
